Financial losses due to health care fraud are in the tens of billions of dollars each year according to the National Health Care Anti-Fraud Association (NHCAA). In fact, losses resulting from fraud, waste and abuse (FWA) are typically estimated to be between 3%-10% of total health care expenditures. This makes it an important topic for employers in terms of both cost and fiduciary plan management, yet it receives relatively little attention.
